Outreach Office Forms and Publications

This publication has been designed to familiarize prospective instructors with our non-credit programs. It will take you, step-by-step, through the planning process to the actual implementation of your course. Information on facilities and University policies and procedures are also contained in this publication.

Anyone interested in enrolling in the LearningPLUS Program is asked to complete and return this application form. The application deadlines are: Fall term – August 1, Spring term – December 1, Summer Term – April 15.

This document is for use by prospective Outreach instructors. A current resume can be submited in place of a Non-Credit Instructor Profile form.

This form provides our office with important information regarding your proposed course. Please complete and submit one form per course.
